Palliative Care- Admission criteria and referrals

To be admitted to the Palliative Care Unit at Parkwood Institute, patients must have a life-limiting illness with a life expectancy of less than three months.

Patients or their family caregiver can submit a referral through one of three ways:

-Through a family doctor 
or

-Through an Ontario Health at Home representative 
or

-By sending a referral directly to St. Joseph’s Parkwood Institute Access Office.
