Rheumatology Centre - What To Expect

Please bring your OHIP card to each visit and present to the receptionist upon arrival.

At your first visit you will be asked to fill out a new patient information form. It is important that the information is complete and accurate for us to provide you with the best care possible. In addition, you will be asked to fill out a Health Assessment Questionnaire (HAQ) at each visit.

Please also come with a list of your medications, ideally from your pharmacy, to each appointment.

At your follow-up visits a follow-up form is required to be filled out, including current medications, and the Health Assessment Questionnaire (HAQ)

These forms help the team to understand how active your disease is to help inform your individualized treatment plan. To help speed up your visit, please consider downloading and filling out these forms prior to your appointment.

We are committed to providing excellent, compassionate care in a timely manner. Even though all patients have scheduled appointments, due to the large volume of patients seen, clinics can sometimes fall behind.  Patients who have flaring of their disease may also be added to clinics as required to meet their needs. We ask for your patience and apologize ahead of time for any inconvenience. We strive to limit delays as much as possible, as we value your time. Waiting times for your appointment may vary so please consider this when choosing your parking (you could be here a minimum of two hours and should park accordingly). 

Perfume and Fragrance Policy
Scented products can cause allergic reactions and respiratory distress for other patients, visitors and staff.

Please do not use any scented products, such as perfume, cologne, aftershave, scented body sprays, lotions, or powders, hair sprays, gels, mousse, hair conditioners or shampoos, or deodorants/anti-perspirants during your visit or stay.

If you are unable to attend a clinic visit, please let the office administrator know as soon as possible so another patient can be accommodated in your place.
