Cardiac Rehabilitation and Secondary Prevention Program - How To Access
Your cardiologist or primary care practitioner (family doctor or nurse practitioner) can refer you to the Cardiac Rehabilitation and Secondary Prevention Program. Once we have received your referral, notification of your intake assessment will be sent to you in the mail. This first visit is an opportunity to meet your nurse and cardiologist. We will do a physical assessment and review your medications, and may schedule you to have an exercise stress test. You will have an opportunity to learn about the program and ask any questions you may have. At the end of this visit, we will make you an appointment to attend a group orientation session.
Your family doctor and referring physician will be kept informed of your progress throughout the program.
