1. Form: Complete the Health Review Form and return to Occupational Health at @email. 
2. Instructions: Review the Health Review Instruction Sheet for additional information. 
3. TB Skin Test: Mandatory.

For New Professional Staff, Residents, and Clinical Fellows

• Policy Compliance: Required every two years.
• Fit-Testing: If not certified for masks 1860, 1860s, 1870+, locate a testing session through Occupational Health via their intranet site (login required via Corporate ID). Please arrive 5 minutes early to your appointment and ensure you are clean shaven (if applicable). 
• Mask Availability: Hospital mask types may change; updates will be communicated.

For Visiting Elective Residents, Clinical Fellows and Medical Affairs Supported Students

• Certification: Ensure your N95 Fit-Testing is current for one of the following masks: 1860, 1860S, and 1870+

All Professional Staff, Residents, and Clinical Fellows must comply with the Influenza Vaccination policy (accessible via Policy Manager with your Corporate ID and password). 

Vaccination: Ensure you are vaccinated. During an outbreak, unvaccinated individuals must wear a procedure mask while in patient care areas.
