Transitional and lifelong care program referral form (PDF)
A physician's referral is required for the TLC program (download the Transitional & Lifelong Care program referral form). Referrals are then scheduled for an appointment with the Physiatrist and/or Nurse Practitioner, who may refer the patients to other team members for discipline-specific assessment and treatment as appropriate.
At the TLC program, Dr. Caitlin Cassidy, a specialist in Physical Medicine and Rehabilitation (Physiatry), works together with other allied health care professionals to provide comprehensive care to those with significant health care needs. The team consists of:
- Nurse Practitioner
- Social Worker
- Occupational Therapist
- Speech Language Pathologist
- Physiotherapist
- Rehabilitation Therapist
- Registered Dietician
- Therapeutic Recreation Specialist
- Administrative Professionals
