How to make the most of your diabetes care visit

  1. Bring a list of questions or concerns
    This will help you to focus and remember everything you need to discuss
  2. Bring all of your medications with you
    Pills, liquids, injectables.  Also bring any nutritional  or herbal supplements and “Over  the Counter” medications
  3. Bring your glucose meter and logbook
    It is very helpful for you to write in explanations for lows or highs, such as exercise, illness etc.
  4. Remember to do your blood work at least one week prior to your diabetes visit
    This will ensure that your results will be available to discuss
  5. Bring along a family member or friend
    They can write notes or help you to remember your next steps
  6. Make your follow up appointment BEFORE you leave
