Inpatient Diabetes Education: Referral Process

  1. If the city-wide nurse is consulted and comes to see the patient, she will fill in the referral for diabetes education (if the patient is coming to St. Joseph’s).
  2. If the city-wide nurse does not see the patient, the hospital staff /physician may complete the Diabetes Education Centre referral form and fax it to us at 519 661-1634.
  3. If the patient is from out of town, he or she should be informed to contact his/her own physician and get a referral to their local diabetes education centre. The hospital physician will also send a dictated note to the patient’s family doctor.
