Our 30 Year Journey of Supporting Older Adults and Caregivers in Southwestern Ontario
Since 1992, St. Joseph’s has been the host organization for the Regional Geriatric Program of Southwestern Ontario (RGP SWO). RGP’s were established across Ontario to provide regional strategic and operational leadership for geriatric health care programs and services. RGP SWO provides regional support to Erie St. Clair and South West regions.
From 2016-2023, St. Joseph’s also provided operational support for the South West Frail Senior Strategy (SWFSS), an initiative focused in the Southwest region only. In 2023, in an effort to align initiatives supporting older adults, and provide clarity in our region, the SWFSS brand was sunsetted and aligned within the RGP SWO. Consistent with other regions in Ontario, we continue to operate as the Regional Geriatric Program, serving all of Southwestern Ontario.
Moving forward in 2025, the Regional Geriatric Program of Southwestern Ontario is excited to develop a new strategic plan, grounded in the RGP’s five key pillars:
- Excellence in Clinical Care
- Capacity Building & Education
- Capacity Planning
- Research & Evaluation
- Advocacy
Together, with partners from across the region, RGP SWO strives to enhance outcomes and experiences for older adults, their caregivers, providers and the healthcare system.
