Parkwood Access
At Parkwood Institute's Main Building we have a centralized process to refer patients for admission to our Complex Care, Rehabilitation, and Specialized Geriatrics Services:inpatient programs, as well as our Geriatric Rehabilitation Day Hospital outpatient program.For information on referring to all other inpatient, outpatient and outreach services click here.
Rehabilitation and Specialized Geriatric Services provide intensive, goal-oriented, integrated rehabilitation for medically stable patients who have needs which cannot be adequately met by community based services.
The Complex Continuing Care program provides low intensity, long duration interprofessional team support for medically stable patients who require health care services that cannot be provided at home or in the community.

Please advise patients you are referring to Parkwood Institute that it is a smoke-free facility. This means there is no smoking indoors or outdoors anywhere on the Parkwood Institute property, including in parking lots. Patients who wish to smoke must do so off the property. Support is available for patients who wish to quit or manage their smoking addiction.
